It's actually fairly low-level. Multipart/related quickly reaches this 
kind of nesting.

I noticed, however, that the mail is broken: The Content-ID fields are 
syntactically broken. They don't contain valid msg-id's (no right-id).

Maybe the bug reporter can write a bug report to the authors of 
IncrediMail ;-) But that's not the reason for KMail not showing it 
correctly. It's simply lack of mulitpart/releated support.
