Probably I'm much too late, but I still like this one most. How about an
additional "don't ask me again" as mentioned and the text field showing the
last selected other option. That means:
By default, the first option is selected and the title is in the edit field
which is disabled. If you select the first or second option the title/url is
put into the disabled edit field. If you select the third option, the edit
field is made editable and you can start with the url OR title and make your
changes.
Hmm, maybe this is not too different from the "fill from ..." solution and
is not too intuitive because you could not see that you can start with the
URL editing if you first select the second option. I guess I just invented
sth. for the "interface hall of shame"... ;-(
