And the back button is actually disabled, i.e. step 2) didn't create a history entry.

Could it be 429446?
The log said

The problem with that is that konqueror doesn't add a history entry, if we don't tell it to....
IIRC khtmlpart should "emit ext->openURLNotify()" for it to happen.
