A symbol that is included both through a normal include and one with ..
(dotdot) is shown twice in the rename refactorings dialog. Subsequently
kdevelop tries to rename this symbol twice and fails (but no crash).

steps to reproduce:

project with content:
/
  foo/
    foo.h
       class Foo {}; 
    bar.h
       #include "foo.h"
       #include "../foo/foo.h"
       void bar() { Foo f; }

Then rename the Foo symbol from bar.h.
