Did you try something like this:

  file_menu = new QPopupMenu;
  file_menu->insertItem(SmallIconSet("filenew"),i18n("&New..."),
  	this,SLOT(slotFileNew()),0,ID_FILE_NEW);
  file_menu->insertItem(SmallIconSet("fileopen"),i18n("&Open..."),
	this, SLOT(slotFileOpen()),0 ,ID_FILE_OPEN);

  menuBar()->insertItem(i18n("&File"), file_menu);
