Open KMail's configuration file (~/.kde/share/config/kmailrc) in an 
editor while KMail is *not* running. Look for a line starting with 

askSave_application/pdf=

Remove this line, save the changes and then start KMail again.

(I think there is a way to do this without editing the configuration 
file, but I haven't found the functionality which allows resetting all 
"Do not ask again" decisions.)
