My approach is to leave stuff in the inbox if it needs
followup, and move it out otherwise; the read/unread
distinction isn't that important for me.
But I should have made clear in my original post that
I'm not really married to the idea of having kmail send
an email (frankly, I don't think that would ever be
implemented). I just want a notification that behaves
in a way that meets those requirements, and in particular
one that works for older threads.

Colored, expanded, eyeballs; none of those work when a
thread isn't within a screen or two of 'current.'
