I'm much in the same boat.  When I "remove duplicates" in an IMAP folder, I 
think what happens is one message _is_ removed, but the remaining one is 
invisible unless I delete the indexes.  I hesitate to blame KMail though, as 
the indexes are specifically created by Dovecot.
