It relies too heavily on current MDI ban. With MDI it would lead to 
altering the main window. Because first you close (by pressing 
alt+f4) all child windows and then one more key press and you closed 
the main window.

Even for tabbed MDI this would be a slight confusion because of (x) in 
tabs.

And another reason -- in such case there would be a need for yet 
another action/shortcut "close the main window (directly)".

And yet another reason ;-) -- Ockham razor (in case of what you are 
proposing ctrl+w would be the same as alt+f4).

Alt+f4 should mean the same as clicking on [x] _on the app window_.
