this doesn't work in all cases .. if it were as simple as this, it would've 
been done ... and moving the quit entry isn't an option either since it 
destroys learnability and breaks with the way menus are ordered.

as for apps asking to quit twice, that would be a bug in those apps. they do 
not need to ask, nor should they be. fix the app.
