I am not sure how this is supposed to look like but it sound great to me. 
Could you post a link to such a web-survey? Also I don't think anyone on this 
list has experience with such web surveys - you will have to take the 
leadership. I suggest, you proceede a little bit until something is visible. 
Then, send a mail to the usability list.
Also, in addition to the survey, you need to write a well illustraded paper 
with the results and post it on the net. Also the results needs to be mailed 
to the corresponding developers.
