You're perfectly right.

The more I think about the problem, the more I have the impression that it
would be a big mistake to use English IDs, either partially or completely.

Is it THAT complicated to have the invoqueHTMLHelp() function call transparently
i18n() ? And have it NOT add the .html extension automatically ?

Application : InvokeHTMLHelp("introduction.html");
.po file (Italian) : introduzione.html
.docbook file (Italian) : <chapter ID ="introduzione">

To me, it sounds the simplest and the more user-friendly way to go.
