The reason why started to think about this was just a back arrow in ktorrent 
that was loaded via KIconLoader. I fixed it by checking for the layout and 
loading "forward" instead of "back" when the layout was reversed.
I was wondering how much these things are an issue in other cases than 
back/forward and if it is that much of a problem so it would justify a more 
comprehensive solution (like providing two icon versions in the icon set and 
picking the right one in the icon loader).
However, if there is anything that can't be fixed globally and has to be 
considered by application developers, it should get a section in the HIG.
We ran into similar problems in Akregator with alignment in
