If I put the above in the Execute line of a .kdelnk file, and I get
"Could not execute program LANG=de"  Does one have to create an
explicit script just to set a variable?  Seems silly.

Did You set the the complete command in quotes?
Try
 "LANG=de LC_ALL=kde KDE_LANG=de kvt"
including the double quotes
