I think this is not a bug.  It's quicker to just update the index.
The user can run "Folder->Compact" on a folder to get rid of the deleted
emails and shrink the mailbox file.

Maybe compacting could be done optionally when kmail is exited  -- even
periodically.  [I, for one, don't actually exit kmail (or logout) very often.]
