No bug. all/all is a kde3 name (kept in servicemenus though iirc).
Use application/octet-stream instead (for files), or
"application/octet-stream,inode/directory" for both files and dirs.
Note however that the konqpopupmenu support for "real" plugins (.so) is untested in kde4,
I re-enabled it only recently and didn't have a chance to test it, so you might have to fix bugs
in it (but not this one :).
