So a PCD file is some kind of directory / folder?

This is more complicated, I think. There exist functionalities like file
thumbnails / previews and parts.
I think for browsing PCDs a kioslave would be interesting, perhaps as a
filter (similar to the way zip and tar.gz files work currently)
For viewing special resolution images I don't think it is a good solution.
Think of e.g. a PictureCD with a directory full of PCD files.
You don't really want to browse into each PCD file separately, do you?

I think that what's image manipulation / viewing tools are for. You may want
to convert your PCDs to another file format than png or jpg.

Is it possible to use a KIOSlave that it returns the content of a virtual
file as a QImage?

I am not really sure about this. KWord / KOffice uses a container file
format (.zip) itself, so you would have to integrate a virtual filesystem
into
another virtual file system? Or do you store only the selected image as
QImage into the KWord document?
