I have updated kpdf from cvs (not in cvs, see link below) with the QOutputDev 
from qpdf and modified the rest of the app to get it working again. 
(XOutputDev kept crashing on me, so I didn't use that).

I have notified the maintainer yesterday of this, I'm still hoping for a 
response, but my main question is if I should commit this ? 

If the maintainer doesn't respond, can I take this program under my wings ?

http://oelewapperke.org/kpdf.tar.bz2 (I packaged the entire directory to avoid 
having to create 5 patches).

I plan to include the other innovation from qpdf ( using fixed-point math 
instead of floating-point, which they claim speeds up the rendering process a 
lot).
