tear-off menus make little sense when there is a toolbar containing the same 
icons, other than being confusing.

Plus there is a technical issue: Some applications like to create menus on 
demand, rather than changing items when the application state changes. 

With a torn-off copy, such an application will not disable/enable menu items 
on state changes, which can lead to potential crashes if the user invokes a 
function that wasn't meant to be invoked in the current application state.
