Does KMainWindow::setHelpMenuEnabled(false) help ?
Then you can create it from scratch.
See also KMainWindow::customHelpMenu().

The above disables the creation of the actions.
Note that you can also use <Menu name="help" noMerge="1">....
to lay out the contents of the help menu yourself.
