Old code, this is not the case anymore, update kdelibs (either branch or HEAD, actually, I fixed it in both). (At the time the reason was: some actions without icons would still make sense in toolbars, like KonqLabelAction and KonqComboAction, so the bad rule was "all actions with icons and all non-basic-KAction w/o icons"). But now any action is shown. If no icon, "unknown" is used when plugging into a toolbar.
