Bugfixes suggested by Michael for my problems: 
- setOutputFileName calls setOutputToFile(bool), as QPrinter does.
- detect that we haven't called preparePrinting when we start to print,
to make it possible to use KPrinter without calling setup(). This is used
by KOffice's preview functionality.
