Log Message:
- sync with the sourceforge version.

In detail:

- removed mutexes from the fileInputStream
- change inputdetector, file is now default, removed other ugly things
- cleanup for windows compiler (M$ VC6) who cannot
  handle variable initialisation correct
- added new putImage method for SDL dynamic resizing
  (hacky will someday be reomved)
- SDL video output updates
- switched some class xx back to struct xx, (M$VC 6 compiler is
  buggy like hell)
- examples cleanup pedantic M$VC compiler cannot handle int main() without
  return.
- added Readme.txt for win32 port.
