Fix a warning with a const_cast, remove dead code that depended on 
autoconf checks.. code that co-existed with stuff that would have failed 
if the checks ever failed, remove a bunch of duplicate include statments, 
and fix some of the horrendous formatting.  And not when gethostbyname 
doesn't work, it's likely the user hasn't set their system up correctly 
(i.e. set a domain name).
