Changes as discussed on the list:

* The group "ListItem" is removed, "MainToolbar" is added

* Support for application specific, themeable icons. The directory share/
  apps/$appname/icons can hold a hierarchy identical to the one under
  share/icons. This former is searched first.

  Install icons in it like this using am_edit:

    datadir=$(kde_datadir)/myapp/icons
    data_ICON = myicon

* Size based loading was already part of the new API, by using negative
  group ids. I introduced 3 macros: KIcon::SizeSmall, KIcon::SizeMedium and 
  KIcon::SizeLarge which are -16. -32 and -48 respectively.

  loadIcon("myapp", KIcon::SizeSmall) will work and returns a 16 pixel icon.

Please let me know if there are still issues left.
