KIconLoader::Small what legacy code is using. It denotes a size.

KIcon::Small is not a size, it's an icon _group_, just like Desktop,
Kicker, ... "Small"-group icons are used in listviews.

I agree it's a little twisted.

No, for toolbar, you must use BarIcon(), which is equivalent to
loadIcon(name, KIcon::Toolbar). It should work.
