Log Message:
* a new generated doc_tree caused a segfault
  by using some old item pointer in QListView mouse events
  now the positions will be recalculated by calling setup()
* without --with-kdoc2 option the QT/KDE documentation has no child
  (there were childs if the user specifies the kdoc-index-dir
   with configure)
* if a setExpandable(false) call was made without any child items on it
  the book icons stayed open
