This is the beginning of digest authentication as per draft three of the
http authentication spec.  To get digest authentication properly working
you need to compile kio_http with -DDO_MD5 and probably link it with libmd
(not sure which oses even come with MD5 stuff by default, so I disabled
this).  It's missing quite a few bits and pieces that are somewhat
important, for now.
