CVS commit by coles: 



- Changed various post-increments to post-increments
- Cache list end() calls when doing for loops
- Much const-correctness
- Fixed a potential bug: an item was being removed from a list, replacing the
  current iterator with what remove() returns - the iterator pointing to what
  was the next item.  Then itr++ in the for loop: so the next item was skipped.
