GUI

some significant gui changes.  Removed off switch (may have to either readd
it or stop the menubar from getting hidden).  Moved the "what's this" button
from its rather strange place next to the sliders to the window bar where
it arguably belongs.  And merged several of the top menus into one "effects"
menu, also reworking some of the do... and fix... routines while I'm at it
to not use hardcoded menu IDs.

Expect several more commits in the same vein.
