CVS commit by pmk: 

More work on the javascript debugger:

- maintain stack frame information about global and eval code (as well as
  functions)
- fixed inaccurate/missing line number information for event handlers and
  <script> tags
- display line nos. along with source
- allow toggling of breakpoints by double-clicking on source line
- various other small fixes
