I've tracked this bug down to the following:

opt.length = 0;

removes all elements from the combobox. After that it's not possible to
add new ones ago. Therefore the bug is most likely somewhere in
khtml/html/html_formimpl.cpp or HTMLCollection.

Feel free to dig into the sources or choose one of the other 100 kjs
reports on bugs.kde.org :)
