I experience this same issue with Ubuntu 13.10 (KDE 4.11.5).  I don't use a
screen saver - just the simple locker.  Even logging out and back in doesn't
seem to get the cursor back, so it does seem to be something pretty
fundamental, but I don't know enough about it to know where the bug should be
reported.  It doesn't happen all the time - just sometimes, and it's annoying
because I have to find a way to safely save everything and restart the computer
with no mouse.
