Martin I run trunk (next 4.6) and this is fixed. In System Settings I changed
the icon size to 48 for the main toolbar and immediately after Applying all
toolbars icons went to size 48, included Kate.

For you so, is it only Kate not taking the global icon size? System Settings
itself should change as soon as you apply a new size.
