OK I wrote a unittest and in fact this is because the host defines
options_configure_keybindings/toolbars twice. Once in ui_standards and once in
shell.rc.
The confusing bit is noMerge="0": this is like not having the attribute, i.e.
the ui_standards.rc file *is* being merged. So the standard actions are there
twice in the resulting xml; at the top and bottom of the menu, and they get
placed under the merge.
You want to either
1) use noMerge="1" to not use ui_standards.rc (but if the above is really all
there is, then I don't see why you would want that), or
2) remove the whole settings menu from host.rc, it's already defined like this
in ui_standards.rc.
