In the Recently Used tab of the kickoff widget, when selecting "Clear Recent
Documents" from the context menu, not all items are removed from the list, but
documents whose file names start with a colon (:) remain.

Colons within file names or the complete path don't cause the problem, it only
occurs when the colon is the first character of the file name.

RecentlyUsedModel::Private::removeExistingItem() in
kdebase/workspace/plasma/desktop/applets/kickoff/core/recentlyusedmodel.cpp
looks like the culprit, but it isn't obvious to me where the problem is
exactly.
