After running a program with krunner, if I want to visit a URL, I have to do so
twice for it to launch Konqueror. The window still closes as expected, but
nothing else happens.

I press Alt-F2, type in konsole, and konsole starts up.
I press Alt-F2, type in google.com, and google fails to show up
I press Alt-F2, type in google.com again, and Konq starts with google.
I press Alt-F2, type in konsole, and konsole starts up.

About 10% of the time, I guess, things work as expected.

This happens if I disable the "Applications" plugin, but leave "Command Line"
enabled, but also if I disable "Command Line" but leave "Applications" enabled.

I can do something similar with only the "Locations" Plugin; so I disable all
plugins except for "Locations". If I go back and forth between google.com and
kde.org, it seems to reject it whenever I enter a different URL, even if the
last one was rejected too. I have to enter the same one twice in a row to get
it to go there.

And occasionally krunner gets wedged and doesn't react to the enter key at all:
the KRunner window doesn't go away. I don't think this has to do with what
plugin you expect to be used. It still reacts to "Escape" as expected, and
seems to start working again under a condition I don't understand quite
quickly.
