I've never seen that in either KDE 3.5.* or 4.*.

But I can see a possibly related problem: If I put a small konsole window in
front of a big konqueror window and quickly move the mouse over konsole for a
few seconds (so that focus is alternating between the two) and then stop doing
that, then both window decorations still keep being redrawn multiple times for
another few seconds. I wouldn't be surprised if this delay had other bad
effects on focus determination. (Thats SVN/HEAD from yesterday evening (IIRC),
compiled with relwithdebinfo target and turned off compositing.) 

But frankly, "Focus Follow Mouse" is so broken in the 4.3 branch that it's hard
to keep the different problems apart.  :-)
