Here are the steps because I accidentaly hit return in the wrong place.

1. Create a new qmake based project.
2. Change the Header extension to .hpp
3. Add a new header in the qmake manager (this also works in the generic New File dialog)
4. Type  test as filename
5. The newly created file is  test.h and not test.hpp.
