I confirm this bug (being it the responsibility of Qt or KDE/KOffice).
An application that allows for "custom" page size should also be able to generated \
PostScript print data that accomodates for custom page sizes (and "BoundingBox").
[There is no need to support *all* known/named paper size formats on the world -- \
that would probably give a list of several hundred -- but adding a few envelopes is \
OK. But in any case, a well supported "custom" page size is essential -- people \
nowadays create crazy-sized documents, and high-end office printers tend to support \
any custom page size for printouts. If we had real custom page size support, it would \
cover all several hundred named page sizes too. To make it really convenient for \
those users needing it, it would suffice to allow users to store their custom page \
sizes in a KDE config file under their self-picked names, like "my-15cm-x-15cm-size"; \
whenever this is used to create a document, the PS print data should be a 15cm x 15cm \
file with an according Bounding Box, i.e. for example "0 0 425 425"].
