If that's the case, then why is there a menu command called 
Message->Forward->As Attachment, and why is this the default behaviour for 
the Forward toolbar button?

Well, then, apparently KMail is a broken, non-compliant mail reader.  All 
messages forwarded using KMail's "As Attachment" show up in KMail as 
attachments, not inline.
