The index file contains the subject, the date, the sender, the 
receiver(s) and some more header fields of each message. Furthermore 
the message status, i.e. read, replied, important, deleted, etc., is 
stored in the index file.

When KMail notices that the index file and the mailbox file (in your 
case the inbox) are out of sync then KMail regenerates the index file 
from the contents of the mailbox file.
