The reason for this is that KMail generates index files to increase the 
performance. The downside of this is that those index files can get out 
of sync with the mbox file if pine changes the mbox file. You should 
convert the mbox folders to maildir folders (provided that pine 
supports maildir). As in maildir folders all messages are stored in a 
separate file the interoperatibility problems between pine and KMail 
are reduced to a minimum.

Another solution would be to delete all index files before starting 
KMail (after you used pine). By this KMail will be forced to regenerate 
the index files.

I'm actually trying to merge my old mail-account using the MUA pine onto another \
