KMail is not very memory effective with huge attachments, this is a known
problem and still hasn't changed, but it is worked on. However besides being
ineffective it should work.

Are you running out of RAM or not?

I just saved a 60MB attachment without a problem (KDE from CVS).
At least all my 512MB RAM really were used for this task and were not only
idle as most of the other time.

Well, I get:

michael@btp8d24:~> ls -l JB*
-rw-r--r-- 1 michael users 19527168 Mär 21 22:10 JB2001
Sternzyklen_111201.doc

On my university computer, a PII 233 with 128MB RAM and KDE 2.2.2 that is.
