Suppose you want your new application to live in Utilities submenu.

Go to K-Menu->System->Menu Editor, and click on Utilities. Click 'New
Item' button, fill the fields and choose an icon. Next time you right
click on some file and choose 'Open with...', you should see the
application you've added.
