Well, have a look at how Unreal Tournament for Linux does it - or Quake 3
Arena ...

They use either XF86-DGA (Direct Graphics Acceleration) which is
independant from the normal X desktop, or you can just make the X window
switch to a modeline that was defined in XF86config, kwintv does this
(defines a TV-compatible (same resolution) modeline in XF86config and
provides a button to switch to that exact resolution).
