The port, modem,... are all right, because you connect to your isp.
Your connection script is also ok because ppp starts on the other end
The problem commes from your ppp setup.

It seems your isp indeed uses CHAP ("<auth chap 05>")
You then have to properly configure CHAP on your end :-)
Sorry I can't help further on that one, I don't use chap, but you might
read the PPP-HOWTO, it can help to understand what happens and how to
configure things manually (without kppp)
