It is probably because KDE is not installed with $KDEDIR=/usr.
If that is the case, you either need to make a link or add it to the
environment variable $KDEDIRS. Or, you can simply copy the icon to:
"$KDEDIR/share/icon/hicolor/<size>/apps".
Or, it is possible that the icon will not appear till you restart KDE.
Note that /usr/share/pixmaps conforms to the XDG standards and KDE does
look in: $KDEDIR/share/pixmaps for icons.
