Why not just check 'X-Spam_Status' starts with 'Yes'?
or if the string 'X-Spam-Status: Yes' exists in the
headers?

Spam Assassin scores emails based on heuristic rules
that add points to the email. There are two types
of rules, those that increase the "spam factor" of an
email, and those that decrease it. (In reality its
a really cool use of a genetic algorithm to determine
the actual points awarded, but that's beside the point.)

By default if an email gets a score of 5 or higher
it's considered Spam. A negative score would indicate
that the piece of email accumulated more points to
reduce its "spam factor" than it did to increase it.

So a negative result from Spam-Assassin means it is
really really not spam.
