Jared Mauch's sysmon software, located at  <ftp://puck.nether.net/pub/jared/>,
should have all the functionality you'll need. installation/configuration
is a breeze.

mmmmm, out of band outage notification.  i'm not sure why you'd want to
use fax as a method of outage notification, but pager is definitely a good
thing.  make sure pages are sent out via TAP, rather than your paging
company's e-mail -> paging gateway.  ;)
